中文摘要: 目的:了解肿瘤专科医院药品不良反应(ADR)发生的特点及规律,为临床合理用药提供参考。方法:对我院2013-2014年上报国家ADR监测系统的157例有效ADR报告进行回顾性分析。结果:157例ADR报告中,患者性别差异不明显,年龄主要集中在61~70岁(49例,31.21%);静脉滴注最易引发ADR(148例,94.27%);引发ADR的药品主要为抗肿瘤药,其中植物类抗肿瘤药所占比例最高(29例,40.28%);血液及造血系统为主要受累系统(92例,35.94%);引发ADR例数前5位的药品分别是紫杉醇注射液(16例,10.19%)、注射用奈达铂(16例,10.19%)、注射用核糖核酸Ⅱ(13例,8.28%)、注射用多西他赛(7例,4.46%)、复方氨基酸(15)双肽(2)注射液(6例,3.82%);ADR分级和转归方面,新的ADR 38例(24.20%),严重的ADR 59例(37.58%),一般的ADR 60例(38.22%),其中痊愈50例,好转102例,未好转5例;严重的ADR以骨髓抑制为主(50例,84.74%),其中紫杉醇注射液发生率最高(14例,23.73%)。结论:临床应加强对抗肿瘤药所致ADR的监测与上报工作,尽可能预防和减少ADR发生,保障患者用药安全。
英文摘要: OBJECTIVE: To investigate the characteristics and regularity of adverse drug reaction (ADR) in cancer hospital in order to provide reference for clinical rational use of drugs.  METHODS: A total of 157 cases of ADR reported by our hospital to the State ADR Monitoring Center from 2013 to 2014 were analyzed retrospective. RESULTS: Among 157 cases of ADR reports, there were no obvious difference between men and women, and the cases mainly occurred from the age of 61 to 70 (49 cases, 31.21%). Ⅳ injection was the most probably way to cause the ADR (148 cases, 94.27%). The main ADR-inducing drugs was antineoplastic drugs, among which the plant-derived antineoplastic drugs caused the large number of ADR reports (29 cases, 40.28%). The hematological system was the main system involved in ADR reports (92 cases, 35.94%). Top 5 ADR-inducing drugs were Paclitaxel injection (16 cases, 10.19%), Nedaplatin for injection (16 cases, 10.19%), Ribonucleic acid Ⅱ for injection (13 cases, 8.28%), Docetaxel for injection (7 cases, 4.46%) and Compound amino acids (15) and dipeptides (2) injection (6 cases, 3.82%). There were 38 cases of new ADRs (24.20%), 59 cases of serious ADRs (37.58%) and 60 cases of general ADRs (38.22%). 50 cases of the ADRs were cured, 102 cases were improved and only 5 cases were not improved. The most majority of serious ADRs were about the myelosuppression (50 cases, 84.74%), while the serious ADRs were firstly induced by Paclitaxel injection (14 cases, 23.73%). CONCLUSIONS: It is important to strengthen ADR monitoring and reporting of antineoplastic drugs and adjuvant therapeutic drugs, in order to most likely prevent and reduce the occurrence of ADR, guarantee the safe use of drugs.
