中文摘要: 目的:探讨氟伐他汀对尿毒症患者炎症因子、营养指标、肾功能的影响。方法: 96例尿毒症患者随机均分为对照组和观察组。对照组患者给予降压、降糖、控制饮食、改善贫血,同时结合血液灌流串联血液透析等常规治疗;观察组患者在对照组治疗的基础上给予氟伐他汀钠胶囊20 mg,口服,每日1次 。两组疗程均为3个月。观察两组患者治疗前后炎症因子[C反应蛋白(CRP)、白细胞介素(IL)-6、肿瘤坏死因子(TNF)-α]水平、营养指标(血浆白蛋白、血红蛋白、血浆前白蛋白)、肾功能水平[血尿素氮(BUN)、血肌酐(Scr)、人全段甲状旁边腺素(iPTH)]水平及不良反应发生情况。结果:治疗前,两组患者炎症因子水平、营养指标、肾功能水平比较,差异均无统计学意义(P>0.05)。治疗后,观察组患者炎症因子水平显著低于同组治疗前及对照组,营养指标显著高于同组治疗前及对照组,差异均有统计学意义(P<0.05),而对照组治疗前后比较差异均无统计学意义(P>0.05);两组患者肾功能水平均显著高于同组治疗前,且观察组高于对照组,差异均有统计学意义(P<0.05)。两组患者治疗期间均未见严重不良反应发生。结论:在常规治疗的基础上,氟伐他汀可降低尿毒症患者炎症水平,改善微炎症状态、营养水平和肾功能,且安全性较好。
英文摘要: OBJECTIVE:To investigate the effect of fluvastation on the serum inflammatory factors, nutrition indicators and renal functions of patients with uremia. METHODS:96 patients with uremia were randomly divided into control group and observation group. Control group was treated with antihypertensive, hypoglycemic, diet, anemia improvement, and hemoperfusion combined with hemodialysis and other conventional treatment; observation group was additionally given 20 mg Fluvastation granule, orally, once a day. The treatment course for both groups was 3 months. Serum inflammatory cytokines[C-reactive protein (CRP), interleukin (IL)-6, tumor necrosis factor (TNF)-α], nutrition indicators (serum albumin, hemoglobin, plasma prealbumin), renal functions [blood urea nitrogen (BUN), serum creatinine (Scr), immunoreactive PTH (iPTH)] before and after treatment, and the incidence of adverse reactions in 2 groups were observed. RESULTS: Before treatment, there were no significant difference in the serum inflammatory cytokines, nutrition indicators and renal functions between 2 groups(P>0.05); after treatment, the serum inflammatory cytokines in observation group were significantly lower than before and control group,nutrition indicators in observation group were significantly higher than before and control group, the differences were statistically significant(P<0.05),there were no significant difference in control group hefore and after treatment(P>0.05); renal functions in 2 groups were significantly higher than before, and observation group was higher than control group, the differences were statistically significant(P<0.05). There were no severe adverse reactions in 2 groups during treatment. CONCLUSIONS:Based on the conventional treatment, fluvastatin can reduce the serum inflammatory cytokines levels, and improve micro-inflammatory state, nutrient levels and renal functions, with good safety.
