| 中文摘要： | 目的：促进静脉用药集中调配中心（PIVAS）医嘱审核工作的规范化。方法：介绍我院PIVAS开展的医嘱审核规范化工作情况，通过比较规范化工作开展前（2016年第二季度）、后（2016年第三季度）药师对不合理医嘱的干预率和医师对药师用药建议的采纳率，评价规范化工作开展的成效。结果：我院PIVAS通过规范审核标准、及时维护和更新药品说明书、建立相关资料库等方法开展了医嘱审核规范化工作，规范化后药师医嘱审核标准一致、依据充分，审核效率提高、审核结果更准确。与规范化工作开展前比较，规范化工作开展后干预率下降[1.41％（308/21 776） vs. 0.74％（168/22 750）]、采纳率上升（95.1％ vs. 98.8％）。结论：我院开展的医嘱审核规范化工作提高了临床医师开具医嘱的合理性及对药师用药建议的接受程度。 |
| 英文摘要： | OBJECTIVE： To promote the standardization of medical orders auditing in Pharmacy intravenous admixture service （PIVAS） of our hospital. METHODS： The standardization work of medical orders auditing developed in PIVAS of our hospital was introduced. According to comparing the intervention rate of pharmacists to the irrational medical orders and acceptance rate recommended by physicians for pharmacists before （the second quarter of 2016） and after （the third quarter of 2016） developing the standardization work， the effectiveness was evaluated. RESULTS： PIVAS of our hospital had developed the standardization work by standardizing auditing criteria， maintaining and updating drug instructions in time and establishing relevant database， etc. After standardization， pharmacists audited the medical orders more accurately and efficiently according to consistent standards and sufficient evidences. Compared with before， the intervention rate was decreased [1.41％（308/21 776） vs. 0.74％（168/22 750）] after developing the standardization work， and acceptance rate was increased （95.1％ vs. 98.8％）. CONCLUSIONS： The standardization work of medical orders auditing developed in PIVAS of our hospital has improved the rationality of clinicians giving medical orders and acceptance degree of pharmacists’ medication recommendation. |
