| 中文摘要： | 目的：系统评价稳心颗粒辅助治疗不稳定型心绞痛的临床疗效和安全性。方法：计算机检索PubMed、Embase、Cochrane图书馆、中国生物医学文献数据库、中国知网、维普、万方等，收集稳心颗粒联合西医常规治疗（试验组）对比单纯西医常规治疗（对照组）用于不稳定型心绞痛的临床随机对照试验（RCT），提取资料并利用Cochrane偏倚风险评估工具5.0.1对纳入研究的方法学质量进行评价后，采用Rev Man 5.2软件进行Meta分析。同时，采用倒漏斗图、Egger’s检验和非参数剪补法评估发表偏倚；采用TSA 0.9软件进行试验序贯分析。结果：共纳入30项RCT，涉及3 405例患者。试验组患者的心绞痛总有效率[RR＝1.21，95％CI（1.17，1.24），P＜0.001]和心电图总有效率[RR＝1.26，95％CI（1.17，1.35），P＜0.001]均显著高于对照组；且不同疗程的亚组分析提示，试验组患者的心绞痛总有效率和心电图总有效率均显著高于对照组（P＜0.05）。另外，两组患者各不良反应发生率比较差异均无统计学意义（P＞0.05）。倒漏斗图和Egger’s线性回归分析结果提示，心绞痛总有效率的相关研究存在一定的发表偏倚，而心电图总有效率相关研究的发表偏倚较小；敏感性分析和剪补法分析表明该结果稳定、可信。试验序贯分析提示，稳心颗粒联合西医常规治疗用于不稳定型心绞痛的疗效证据确切。结论：稳心颗粒联合西医常规治疗用于不稳定型心绞痛安全、有效。 |
| 英文摘要： | OBJECTIVE： To evaluate the clinical efficacy and safety of Wenxin granules in the adjuvant treatment of unstable angina pectoris （UAP）. METHODS： Retrieved from PubMed， Embase， Cochrane library， CBM， CNKI， VIP， Wanfang database， etc.， randomized controlled trials （RCTs） about Wenxin granules combined with routine Western medicine （trial group） versus routine Western medicine （control group） in the treatment of UAP were collected. The methodological quality of included studies was evaluated by using Cochrane bias risk assessment tool 5.0.1. The data were extracted and Meta-analyzed was performed on by using Rev Man 5.2 software. Funnel plot， Egger’s test and non-parameter trim and filling method were used to evaluate publication bias. TSA 0.9 software was used for trial sequential analysis （TSA）. RESULTS： A total of 30 RCTs were included， involving 3 405 patients. Total response rate of angina pectoris [RR＝1.21， 95％CI（1.17，1.24），P＜0.001] and ECG [RR＝1.26， 95％CI（1.17，1.35）， P＜0.001）] in trial group were significantly better than control group. Subgroup analysis of different treatment courses showed that total response rate of angina pectoris and ECG in trial group were significantly higher than control group （P＜0.05）. There was no statistical significance in the incidence of ADR （P＞0.05）. The results of Funnel plot and Egger’s linear regression showed that there was certain publication bias in related studies about total response rate of angina pectoris， but the ECG total response rate related research publication bias is small. The results of sensitivity analysis and trim and filling method were stable and reliable. TSA suggested that there was definite therapeutic efficacy evidence for Wenxin granules combined with routine Western medicine in the treatment of UAP. CONCLUSIONS： Wenxin granules combined with routine Western medicine is safe and effective for UAP. |
