| 中文摘要： | 目的：探讨临床药师在帕金森合并脑梗死患者药物治疗中的作用。方法：临床药师参与1例帕金森病合并脑梗死患者的治疗，从帕金森病、脑梗死及高血压等因素进行分析并提出药物治疗建议，并为患者制订出院带药卡。患者使用多巴丝肼后出现幻觉、焦虑等精神障碍时，临床药师建议给予盐酸普拉克索0.125 mg，po，tid治疗精神障碍；多巴丝肼减至125 mg，po，tid；给予厄贝沙坦150 mg，po，qd与氢氯噻嗪25 mg，po，qd控制血压。结果：医师采纳临床药师建议。患者经过治疗后，帕金森症状明显改善，血压控制良好，病情好转出院。结论：临床药师参与多项疾病合并患者药物治疗方案的制订，可协助医师完善用药方案，提高用药疗效和安全性。 |
| 英文摘要： | OBJECTIVE： To discuss the role of clinical pharmacist in drug therapy for patients with Parkinson’s disease and cerebral infarction. METHODS： Clinical pharmacists participated in drug therapy for a patient with Parkinson’s disease and cerebral infarction； analyzed the factors as Parkinson’s disease， cerebral infarction and hypertension； put forward drug therapy suggestions and developed discharged medication card. When the patient suffered from psychotic disorder such as illusion and anxiety after treated with madopa， clinical pharmacists suggested using pramipexole dihydrochloride 0.125 mg， po， tid to treat psychotic disorder； madopa 125 mg， po， tid； irbesartan 150 mg， po， qd and hydrochlorothiazide 25 mg， po， qd for blood pressure control. RESULTS： The physicians adopted the suggestions of clinical pharmacists. Parkinsonian symptoms was improved， and blood pressure was controlled well after treatment. The patient was discharged from the hospital after recovery. CONCLUSIONS： Clinical pharmacists participate in drug therapy for patients with multiple diseases and assist physicians to improve medication regimen to improve therapeutic efficacy and the safety of drug use. |
